A few studies on medicinal prophylaxis of papulopustular exanthema have been performed; their results do not allow for reliable recommendations. In a placebocontrolled, double-blind study the administration of tetracycline (500 mg twice daily) had no significant effect on the incidence of cutaneous lesions in the first 28 days of EGFR blockade [31 ]. An eight-week administration of minocycline reduced in a small placebo-controlled study the number of lesions in the face and showed a trend to reduced frequency of moderate to severe papulopustular exanthema (week 4: 20 % vs. 42 %; p = 0.13). The additional application of tazarotene in the face proved to be of no clinical benefit, but was associated with significant irritation [32 ]. The prophylactic use of topical pimecrolimus in the one half of the face in patients receiving cetuximab therapy showed not significant advantage [33 ]. In the phase II study STEPP the prophylactic 6-week treatment with moisturizer, sun protection agent, topical glucocorticosteroid (1 % hydrocortisone cream) and doxycycline (100 mg twice daily) starting on day 1 before the initial administration of panitumumab reduced the incidence of skin toxicities of grade _ 2. During the 6-week study duration 29 % of patients in the prophylaxis arm developed severe cutaneous side effects as opposed to 62 % in treatment of already manifest skin reactions [34 ].
